Fall prevention should be the goal of many hospitals. Falls occur every year in hospitals and can be harmful to the patient, especially the elderly (Peel, Travers, Bell, & Smith, 2010). Falls can lead to broken bones, longer hospital stays and potentially lead to death. Falls are costly to both the patient and the hospital (Trepanier & Hilsenbeck, 2014). This document would like to take a look at a strategic action plan that would help prevent falls. This action plan will include organizational changes geared towards preventing falls. The second part of this document will include an evaluation plan designed to measure and monitor the plan. Strategic Action Plan Assessing the reason for the falls would be the first step of the plan. Is there any repetition of the reason behind the fall (Olrich, Kalman, & Nigolian, 2012)? For example, are there frequent falls at certain times of the day, such as during shift change? It may also be helpful to know if the fall was related to attachments such as IV poles or Foley catheters. It might also be a good idea to look at the type of patient who fell: was the patient confused at the time of the fall? Was there a history of dementia reported for the fallen patient? Once all the information is gathered, the relevant information could be classified into groups that would help decipher the best action plan to prevent falls. The plan should be written and approved by nursing managers and could be implemented by clinical managers involved in the unit. Although each unit is different, some ways to prevent falls are similar across all units of the hospital and a team should be formed to use the information… at the center of the paper… the importance of fall prevention for patients admitted to the department (Roussel, 2013). Further evaluation would establish whether the plan set was successful in reducing the number of falls or whether the plan needs to be revised. If changes need to be made, the written guidelines should be reviewed and shared with staff so that the review can take place. Fall Prevention Results It is the hope and goal of many hospital staff members to help reduce the number of falls in hospital settings. The hope is to establish a plan that will help nursing staff decrease the number of falls. Falls can be extremely harmful to older adults. Preventing falls is a much-needed goal that will bring better outcomes for the patient and the hospital. Evaluation of the action plan will also be planned so that necessary revisions can be made to reduce the number of patient falls..
